If you rent without a lease (on a month to month basis) your landlord must fill out this form and it must be signed in the presence of a Notary Public.If you reside with a Valley Central School District Resident (you do not own or rent your own home) BOTH of these affidavits (#1 and #2) must be completed and both must be signed in the presence of a Notary Public. Affidavit #1 gets filled out by the Parent of the child applying for UPK and Affidavit #2 gets filled out by the Valley Central School District Resident.
